Veneral Warts are sexually transmitted warts that occur on the penis , on the bone above the penis, and anal skin and on the thighs. The risk of skin cancer is rare. Just as in women the cause is HPV- human papilloma virus. There are several approaches for treatment. First men should be tested for other sesually transmitted diseases such as gonorrhea, syphilus, and hiv. Warts can be treated at home with either condylox or aldara. Condylox, an extract of of a plant, is used for three days, twice a day. The area is allowed to rest for 4 days. This can be repeated for 4 weekly cycles with an overall potential cure rate of 85%. Care should be taken to only treat the warts and not normal skin. Aldara is applied three times a week to the whole area for 4 weeks In men. Aldara works in only 38 per cent of cases. For those resistant to either medicine, in office treatments include freezing, electrosurgery, laser, or an acid solution. ( podophyllin). Often it can require several visits over a few months to eradicate the warts. Any treatment may damage the skin color. During and after treatment prevention with a latex condom is essential.Men must consider themselves contagious even when there are no visible warts and their partners must be checked by a gynecologist to prevent retransmission back to the male.

A useful approach for the face is injectable fillers. Fillers are used traditionally in the fold between the mouth and nose- the nasolablial fold. Other areas treated are the areas under the corners of the mouth- marionette lines. Heavy folds need to be evaluated from the perspective of the cheek.As people age, the normally rounded cheek flattens and shifts toward the fold making the fold deeper. If only the fold is injected it can not be totally corrected. A better approach is to rebuild the cheek and lift the fold. Filler can be injected under the zygoma the bone underneath the eye safely. This is a injectable cheek or mid-face lift. Lmitations are the volume necessary and how long the filler lasts. Artefill is well suited for this use because it lasts at least 5 years. The area can be corrected in 2 sessions gradually without the obvious lumpiness of fat transplants or cheek implants. The material is soft , is not prone to bruising ,and does not lump. The look is very natural and peasing and lasts for years.

laser hair removal has been around since 1996. Lasers currently used are the alex, yag, diode and ipls( intense pulse light). Lighter skin colors do best with an alex or diode. Darker skin does best with the nd-yag laser. A limitation of removal of hair on womens faces has always been the effect of abonormal hormones. Women with polycysitc ovaries find it difficlut to totally remove lip, chin, neck and sidedburn hair. The extra male hormone stimulates hair growth. Spas and non-medical facilities will find it difficult to treat these women. Even when blood tests are normal a tiny amount of male hormone will make these women laser resistant.The solution is to combine a medical treatment form the outside or prescribe low dose water pills to weaken the efffects of the hormones. Typically, 2-3 months before laser treatment these medical approaches should be started. Waxing can also increase the effectiveness of laser treatment by waxing 2 weeks prior to laser sessions so all the hairs in the area are growing at the same time. These women may still require maintenance sessions about 3 months apart to maintain the effect.With this medical approach to laser hair removal better results are commonly achieved.

In the removal of localized fat there are two approaches one surgical and one chemical. Liposuction which has been used for 25 years involves injection of large amount of diluted anesthetic and the introduction of instruments to remove fat. It causes a lot of swelling and a few small scars. it can be used on the belly, saddlebags neck etc. At times it can reslut in uneven appearance and if a large amount of fat is removed you can gain fat in other areas. About one third experience an increase in breast size. The swelling usually lasts for weeks and traditionally a compression garment is worn. For smaller areas liposuction has been replaced by lasers which dissolve the fat. This device leaves smaller scars but can not remove large areas of fat. Mesotherapy is a European technique for localized fat removal using chemical injections. it requires 3-4 sessions spaced weeks apart. it cna be used on multiple small areas where liposuction is not approriate. It can be used as a touch up to liposuction. It results in localized swelling for one week each session. it requires a commitment of aobout 3-6 months to reach the desired endpoint. It is also not recommended for very heavy people.It is less expensive than laser or traditional liposuction.

Often people with acne can be left with permanent depressed scars. Trauma or surgery can also leave depressed or indented scars. Recently Laser has become the approach to improving depressed scars. Either a single session with a carbon dioxide laser which will result in days of downtime and potential months of redness or multiple treatments with non-ablative lasers can improve scars. Available lasers include Fraxel Co2, fraxel erbium, and Nd Yag lasers. The nd yag has no downtime and results in permanent improvement of 35-40 per cent. The fraxel erbium does leave days of redness and results in up to 50 per cent improvement. The treatments are more expensive costing about 4-5,000 for a series of treatments versus 1,250 for yag. Combining yag with a long term filler such as Artefill can give a significant long term improvement at a lower cost than Fraxel.

Stretch marks occur both in men and womem starting as teeenagers with growth spurts. Fresh stretch marks are red while older stretch marks are white. Various cream including Strivectin have been used on stretch marks with variable results. Thre laser systems starting with the pulse dye laser system have been used for stretch marks with a mild improvement in color. The excimer laser temporarily improves the color of light strrtch marks.There are claims for expensive Fraxel laser treatments but there are no credible before and after pictures on the manufactueres website. We dsecribe two treatments for stretch marks that give postive long lasting results.

There are various laser and non-laser reomval techniques. Some are proven effective. while others may even cause scarring.Laser tattoo removal requires specialized equipment- a q-switched laser. There are 3 kinds- ruby, alexandritr, and nd-yag. The ruby can possibly damage darker skin. The alexandrite treats black and dark green well. the nd-yag especially the medlite c6 treats all skin colors but may have trouble treating some dark green colors. The internet products are unproven and can cause scaring. they are variations on acid solutions.
