There's new hope for acne sufferers. At last, a significant scientific breakthrough has created an effective non surgical, medical treatment for resistant, hard to treat acne. This revolutionary treatment is called photodynamic therapy. It uses a special light and a light activated natural solution that targets and destroys acne. Levulan is a 20% solution of Aminolevulin acid ( ALA). ALA is a natural product found in all humans throughout the body. It is needed for the manufacture of hemoglobin. You would not be able to make red blood cells to carry oxygen without ALA in your body.
What causes Acne?
Acne results from the obstruction and irritation of the oil glands in the skin and it affects 80% of people, at various times. Acne typically begins in teenagers, but there are many in their 20s and even thirties who still suffer from acne. Acne can range from mild (blackheads and whiteheads) to pustular, and even severe cystic acne.
Acne is cause by overactive oil glands. The glands, triggered by the hormone androgen, produce (sebum) oil in the skin. Although androgen is a male hormone, it is present in both men and women.
When hormone levels increase, the oil production increases. With clogged pores, oil is trapped beneath the skin causing whiteheads and blackheads. Oil can mix with trapped cells and cause bacteria to grow, causing red bumps that can be sensitive to touch. Severe acne is characterized by large painful lumps and cysts- deep pus filled areas that can cause scarring.
Before Photodynamic Acne Treatment, the best available treatment for resistance and cystic acne was Accutance. However, with Accutance there are many side effects including birth defects, possible liver changes, mood depression, dryness and night vision changes. PDT Acne treatment provides an alternative to all types of acne without having negative internal effects.
The Levulan which is a clear painless solution is applied to the skin after degreasing the skin with a drying solution. Levulan is more effective on dry skin. The Levulan is left on for 60 minutes. It is then activated with 3 full cycles of a special blue light (Multiclear). This takes about 5 minutes.
The Levulan is absorbed by the enlarged oil glands, making the acne cells more sensitive to the light. The combined solution and the light is what gives rise to the term photo-dynamic therapy. The treatment shrinks the oil glands, bringing chronic acne to the surface, despite failures with previous treatments. The treatment reduces oiliness, and minimizes the appearance of pore size. The skin texture improves.
ADVANTAGES
The advantages are a rapid predictable treatment of acne all over the face, without scarring, surgery, or internal side effects. The skin appears refreshed after 3-5 treatments. Treatments are 2,3,4 weeks apart.
DOWNSIDE- Does it hurt?
The downside is that your skin will be red, and broken out for a few days after the treatment. With each treatment, generally there is a 50% improement until the face is clear and stays clear.
How effective is Levulan?
Repeatedly, I have seen women in their 20's and even 30's, who have failed on prescription creams, antibiotics, or even Accutance clear with Levulan.
Are there side effects? Can photodynamic therapy cause permanent damage? Can it cause skin cancer?
No. PDT (photdynamic therapy ) is in fact recommended in the January 2007 Journal of the American Academy of Dermatology as a highly efective first line treatment for actinic keratosis, a common pre-cancerous skin condition which affects 10 million Americans. It is estimated that 40 per cent of Squamous Cell Carcinoma skin cancers develop from these early lesions. European regulatory agencies approved PDT as safe and effective for treating precancerous lesions as well as certain common superficial (thin) skin cancers.PDT is a non-surgical, non-scarring alternative which may reduce future skin cancers in even high risk patients.So rather than there being a risk of this treatment there is the added benefit of removing undetected sun damage without surgery.

Acne Scars
Acne causes damage to the skin in many adolescents and adults with depressed areas of various shapes, rolling, icepick, or boxcar scars. Traditional treatments have involved surgery- originally starting in the 1950's with dermabrasion, where the skin was removed with a wire brush or sanding device. This has been replaced with the Carbon Dioxide or Erbium Laser. Both these ablative lasers involve removing skin with a long healing time, and months of redness.
Is there another approach?
Yes. Recently non -ablative lasers were introduced. Various lasers including smoothbeam, Fraxel and the YAG lasers have been shown to improve acne scars over a number of months with repeated treatments which do not burn off the skin. They stimulate collagen tissue with repeated use. Each of these treatments are about 3-4 weeks apart. We have used both the smoothbeam and YAG and found the YAG results (Medlite laser) to be far more comfortable than the smooth beam. We also have noted that the Medalite YAG yields comparable results to FRAXEL at a far lower cost to the patient. Fraxel treatments can cost thousands of dollars. The Medlite C6 is a safe comfortable alternative with no downtime.
Will the scars go away?
No, none of these treatments will remove deep scars 100%, even the CO2 (carbon dioxide). Laser is only 50% effective.
The Medlite Yag laser can safely, and comfortably show a permanent improvement. In selected patients other treatments can be added to further improve the outcome such as TCA (trichloracetic acid) for ice pick scars, subcision (undermining) depressed scars with a needle to lift the scar or fillers to raise the area. 2 Fillers are temporary last 6-12 months, but can maximize the improvement offered by laser, if desired.
